Introduction

Equotient for wellness is a mental health organisation who have their unique therapy based methods for patients seeking support to come out of different kinds of addiction . Equotient For Wellness was looking for a solution which could address their unique workflow requirements as  they found the conventional products in the market were not flexible enough by design to serve their purpose.

Deliverables

Phases of the patient journey was defined as follows (representative):

Trigger Event/Awareness: The patients review their own symptoms, conduct research, consider potential health conditions that may require treatment, and may even reach out to communities (e.g., posting questions on social media);

Help: The patient makes initial contact with a health centre , including health insurance, via call center, chat, email, mobile, or an in-person visit; 

Care: The patient is evaluated at a facility;

Treatment:  The system provides the patient with both on-site and follow-up care (e.g., counseling, or suggested lifestyle changes);

Behavioral/Lifestyle Change: The patient makes changes to daily routines and takes part in proactive healing in order to prevent relapse and promote long-term wellbeing;

Ongoing Care/Proactive Health: The patient manages his or her care between sessions; meanwhile, the system facilitates engagement between the patient and therapist in order to enable the patient to address symptoms and maintain good health.
